My information is from Friday evening when I was in London and saw the softback on the shelves of Foyles' Bookshop.

The publication date of this edition was 2005.

No I don't but it can be found in UK National Archives file AIR40/3114, report CSDIC CMF (Air) A.503

I think there was a second report on him as well but I was looking for other information and just made a note of this reference in passing.
