RAF victory registration - a special form?
 
I have just recently recieved a bundle of copies of ORB Form 540/541 from UK National Archives. A bit expensive in the long run, but brilliant service from their part.

A have found a number of entries in these stating individual pilot victories, but would they have used a specific form for the registration and confirmation of victories. If significant we are talking Sqns 33 and 274, North Africa, 1941-43 and 234, Britain, 1942.

Generally there was a RAF Form F that was submitted with a claim, but I have seen reports filled on Sortie Report Forms. I do not know if it was due to a requirement or simply lack of proper forms.
The Form F was generally filled by Intelligence Officer, who was the first instance to asses validity of claim. Then the report was sent to command and there it was processed. Finally, the pilot received information of the claim was approoved or not, in one specific case I am awared of, it took about two months during the 1944/45 Winter.
RAF Form Fs are available in AIR50 and ORB appendices at AIR27. There are substantial gaps for MTO, so you may ask Frank Olynyk if there are the ones you are interested in.
