III/KG 40 and GQM

Hi,
can anybody help me with an entry from the General Quartiermeister Luftwaffe dated 12 October 1941. I’ve got a copy on file but it’s almost unreadable.
There’s an entry (number 3) on a He 111 H-6 of III/KG 40. I suspect it is Wnr.4396 or 4996, crew of Lt. Walter Reitenberger, If I’m correct
I’m looking for the correct Wnr, date and names of the crew.
