B-24H 'Malfunction-Junction'

B-24H 'Malfunction-Junction' ...........

I'm looking for any info regarding a ship going by the above name. I believe the B-24 in question was an 'H' model and was stationed with the 489th BG.

I'd be grateful to find any photos of her, especially nose-art, etc.
