Clarification needed for the following Luftwaffe Loss Report:

4/KG51, Ju88A, Engine failure and crashed on force-landing at Bric-Conte-Robert, near Paris, France. Lt. Heinich killed, 3 injured. Aircraft a write-off.
I have: Uffz. Josef Becher (Bm) killed. Lt. Heinich unhurt ??
Does anyone know all the crew names and fates?
