What caused the crash of B-24, Serial # : 42-100281, crashed at July 19th 1944 in Black Forest, Germany

Hi All,
presently I´m doing some research reg. the crash of the above mentioned B-24, flown that day by 2nd Lt. David C. Deeter, 389BG,566BS.

Read some documents and got the info that the ship was flowing in formation and close after bombed the target it was hit by a bomb door which was lost by another B-24.
Because of the damages (one engine failed) the crew tried to make it to Switzerland but hit by Flak (AAA) app. 10 miles before Swiss border and crashed. All crew member survived.
Now my point, is there any document which proves the cause of crash. It is not included in the MACR so for I know.
In addition I´m looking for pics of the crew member. Could find a pic of pilot Deeter but nothing about other crew members.
