Possibly:

Ju 88A-5, WNr 4329, II./LG 1, 5/21/1941, Flak, Gen.Qu.6.Abt. (mfm #3)-Vol.4; Taghon, LG 1, I, p.455, Kreta, 20%.

Note, however the damage is 20%, not shot down, and the location is Crete.
