Following extract (p49) from 219's Sqdn history sheds light :
'Pilot Officer Carroll, who in July had been forced to abandon his aircraft, once again found himself on the end of a parachute harness on 19th August, when Beaufighter X8219 suffered elevator failure whilst approximately forty miles out from the Northumbrian coast. Though on the previous occasion Carroll's crewman survived the incident, sadly on this occasion P/O Kendrick failed to get out of the aircraft. Having no known grave, his name is inscribed on the Runnymede Memorial.'
