Pilots & emergency parachuting

I'm looking for any informations on pilots who had to make more than five emergency parachutings. (in case of technical probs, or shot down,...)
Who held the record in emergency parachuting?
(Axis side & Allied side)
Who held the record in being shot down?
(Axis side & Allied side)

I have read (maybe an urban legend; nevertheless sounds good) that one pilot received after his 11th(?) emergency para-jump the parachutists' badge honorarily.
