The page appears to be a single page from a multi-page MIS-X (Escape/Evader) interview. Apparently, only this page was eventually added to the MACR. Note item 1 identifies the "Source" as Eiff. This format (with reference to the "Source") is typical of how these E & E interviews were done. (The misspellings can possibly be attributed to how Eiff remembered the names.)
This list of MIS-X interviews : http://www.conscript-heroes.com/esca...-Fort-Hunt.htm shows the Eiff interview as number 371 (on page 4 of the series). These interviews on page 4 are not on-line (as the first 3 pages are) but can be ordered on CDs from the Air Force Historical Research Agency. Eiff's is on CD A1315 .
