Melvin,

You are right, the name is Josef Forst. In the death notice there only is mentioned "Unglücksfall" (accident) as reason for why he passed. Only a guess: If the accident did happen while this guy was not aboard a LW-aircraft but rather on the ground (i.e. car accident) there will be no entry recorded in any LW loss list like NVM or GQM.
