Crippled Skyhawk

Hi all:

Whilst trawling various books and magazines for battle-damage pictures I came across a picture in a book called Air War Over Vietnam Vol III by Dana Bell (Warbirds Illustrated No.21), showing an A-4E of Attack Squadron 212 ("Rampart Raiders") returning to the USS Bonhomme Richard after taking hits from AAA over North Vietnam. Date April 1967.

In the picture the Skyhawk has only the nosewheel and the arrester hook down, and the whole rudder has been shot away.

Can anyone put a date and a name to the incident, and confirm that the pilot (presumably) ejected safely ?
