Crashlanded HS 126 T1+AK

Does anybody have details of a crashlanding of a Hs 126 with Kennzeichen T1+AK? Looking for a date and eventually crew details.

1941-06-29, 2.(H)/10, Hs 126A-1, 3031, T1+AK, Krystynopol-Brody-Podkamien-Busk-Toporow, Unbekannt. Bruch 100 %.
Flugzeugführer Uffz Kavka, Ottokar, tot
Beobachter Ofw Metz, Friedrich, tot
1941-07-25, 2.(H)/10, Hs 126B-1, 4194, T1+AK, Fl.Pl. Shitomir, Absturz nach Start. Bruch 100 %.
Flugzeugführer Gefr Hübner, Heinz, verletzt
Beobachter Fw Billker, Otto, verletzt (Am 1941-07-26 Fw Willker seinen Verletzungen erlegen)

T1+AK was lost also in air collision in April 1940 (source Unfallliste) but it rather crashed into the sea.
John we need more details (1940 or later?, estimation of % damage etc.).

Uploaded the picture, I hope it works.... I believe this picture was taken during initial phase of Barbarossa....
