Loss of Fw 189 of 5.(H)/12 on July 01, 1942

Anyone has further information about the loss of a Fw 189 of 5.(H)/12 on 01.07.1942, please ? The observer Leutnant Edwin Bader took over control of the machine and belly-landed the machine after the pilot and the gunner having been fatally wounded.
I am looking for the names of the pilot and gunner, as well as the marking and Werknummer of the plane.

By the way, Bader was a Deutsches Kreuz in Gold recipient.

Thank you GRM, this is exactly what I was looking for!

Just as addition:
Deutsche Kreuz in Gold was awarded to Bader (then Oberleutnant) on Febr. 19, 1943 and Ritterkreuz on March 26, 1944.
Originally an observer, he was transferred in mid 1944 to receive pilot's training (as many former observer did at that time), but was killed in an accident, probably on a training flight, near Frankfurt/Oder on July 12, 1944.
Circumstances and type of aircraft not known to me.

1944-07-12, FFS A 41, Kl 35 B, 1607, RT+BC, Finkenheerd bei Frankfurt, Zusammenstoß in der Luft mit Kl 35B, Werk.-Nr. 4571. Bruch 100 %.
