Hi all,

I am working on a new book - project RAF accidents of WW2 that resulted from human error, this subject is quite a big thing in the modern military, but actually was very apparent during WW2. Therefore if anyone can assit with.

- WW2 RAF - Commonwealth accidents that occured that have a paper trail - accident - information of interest, researchable etc for a reader. Can examples on the ground and in the air, falling asleep is one that comes to mind!

Abit random, but any help or leads would be good.

This is a nice subject Danny...

I do have the 2 complete SETS of the TEE EMM and after years on the shelves, I started to read the First complete volume this year...

This is a "heart subject" as well since I worked as a Flight Instructor for years (civilian aviation) and completed the "Aircraft Accident Investigation Course" at CENIPA, the "Brazilian Air Force" equivalent of the "Air Accidentes Investigation Branch".

Well, I can say that SEVERAL monthly magazines of TEE EMM do have samples of Human Error accidents....I was just reading one of a guy who beat-up an airfield, collided and crashed...killing himself and a dozen WAAF and Ground Personnel on one Hangar....

On some War Diaries you will find quite a number of those...but surely you will need to read or find the Accident Investigation Board Reports....the originals....

I do recommend you the book QIUETUS - Accident proneness in the RCAF....written by a friend, named Anne Gaffiuk, from Canadá...She did a mervellous work selecting some accidents and the Investigation Board findings. Fully recommended reading...it is fantastic.

Reading the F1180 accident record card, you get the impression that the powers that be were very ready to blame the pilots and crew

I have never seen them ask the question, what were the root causes of that failure - lack of information, instruction, training, supervision, workload, command expectations etc.

It is easy to blame the crew, harder to blame yourself for putting him there in the first place

Martin-this is why aircraft accidents are such an emotive subject-look at the fallout following the Mull of Kintyre Chinook accident

I also do recommend you the following serie of books from Ross McNeill:

I sadly do have only the Volume 1, but you will find quite a number of entries for mishaps and not followed standard procedures (sadly also they did not have at that time the SOP, although they did have procedures to follow)...TRIM TAB adjustements for instance claimed quite a lot of airplanes...Navigation errors or mistakes (particularly with Fog and poor weather), etc., etc....

Fully recommended reading....really....it is FANTASTIC and well done....

Maybe you can sort out some samples of those accidents by SUBJECT....A full list of them, for the period of 1939-45 would be "Mammoth" though....

I can also recommend the books by Colin Cummings.

Most likely a myriad of "self-inflicted" accidents inside his volumes, especially those dealing with training
