11 November 1941 - 56 Squadron Typhoon crash

Dear all,

Am looking for details about the NAME of the pilot and serial number related to the very first casualty with the Hawker Typhoon Mark Ia from Nº56 Squadron, on the 11th November 1941.

It is said that the pilot died by CO2 getting into the cockpit and that a huge crash investigation was made.

Is that CRASH Investigation available somewhere? If yes, where?
