Regarding the Bat-21 episode of the Vietnam War

How was the EB-66 shot down if it should have been able to identify and jam missile frequencies? Did the NVA use a new, hitherto unknown, system?
I read two EB-66s were in that sortie.

Is there an assessment report on the incident?

There is a detailed assessment that "may" fall into the 'more than I wanted to know' category. It is long.
