is anyone able to help with following questions? I would like to know the exact date of his USN joining and also when he left the USN.

1135 – Line officer under instruction as a prospective staff corps officer, USNR
2205 – An officer in the Dental Corps, USNR

He evidently left active service to attend medical/dentistry school and rejoined the reserves as an Ensign before he got his oral surgeon degree
(for he did, indeed, become an oral surgeon) and was subsequently promoted to Lieutenant (JG), DC.
