Pilot document question.

I have a question related to a document describing an pilot from 356FG,

"387th Fighter Squadron, code named "Bluebird", 365th Fighter group “Hell Hawks” P-47 , B4 marking. Matusiewicz, Daniel 1st Lt. Chicopee, MA 7/43 to 11/44 (DFC) (18 BOLC) RK: 1LT; serial: AO00680142; unit: 387FTR; ECH: SQ; Theater: ETO; CR: 0,5; Date: 440625;"

What does 18 from 18 BOLC stands for, and what are the menings of the dates 7/43 to 11/44 and Date: 440625?
